Annual CEO Evaluation - Direct Reports Questionnaire


SIGA Board Management Policy B04-002 states that a President/CEO performance evaluation will be done annually. Members of the Board have been asked to complete a questionnaire rating the CEO's achievement of goals and priorities and adherence to management policies. Findings will be used to determine variable pay.

This questionnaire is to be completed by direct reports to the CEO and is designed to gather your perceptions of the CEO's leadership of the organization. Results are not factored in to the variable pay calculation but will be used to provide constructive feedback to the CEO. Your findings will be confidential and will be combined with other responses received to ensure anonymity.

You can complete this below and it will go directly to the external consultant. You will have a chance to edit your responses after you press "Submit". Your responses must be received by Wednesday, January 30, 2008.
