Management Team Self-Assessment Questionnaire


This on-line questionnaire is designed to gather information about the current level of team functioning among management staff at the various Riverside Sites. Results will be used to shape the content of our team development workshop scheduled for January 6 and 7, 2009.

Please complete this by Wednesday, December 17, 2008. You will have a chance to edit your responses when you click "Submit" Your survey will go directly to the external consultant facilitating our January session, so your replies will remain completely anonymous and confidential.
