Ontario Heart Health Network Strategic Planning Process


The Ontario Heart Health Network (OHHN) has initiated a strategic planning process to identify future directions and priorities. Part of this process involves obtaining the views of our members and key stakeholders about the various issues and choices facing the organization.

Please complete this questionnaire no later than Friday, July 30, 2004. The survey findings will be used at a planning retreat to be held in the fall.


1. The OHHN's current mission statement reads as follows:

To promote and support opportunities for those involved in the planning and delivery of community-based heart health programs to learn, share and network with their peers.
