LHIN 4 Human Services And Justice Coordinating Committee
Survey of Service Providers and Community Organizations


The LHIN 4 Regional Human Service and Justice Coordinating Committee is conducting a needs analysis to aid in its planning and your views are an important part of this process.

Human Services and Justice Coordinating Committees (HSJCC) have been established by multiple ministries to develop an integrated system of services for individuals with mental illness and other disabilities who are involved with the justice system (courts, jails, and police). The HSJCC's are organized into provincial, regional and local tiers. The LHIN 4 HSJCC is the regional body, with jurisdiction over four local committees from Brant, Haldimand and Norfolk, Hamilton, and Niagara.

Please complete this questionnaire no later than 5:00 p.m. Wednesday, March 28, 2007. Questionnaires will go directly to the external consultants, so only the researchers will see your replies.

When you complete the survey you will be able to review and edit your responses before they are submitted.
