This on-line questionnaire is designed to gather information about the current level of team functioning of the HIROC management team. Results will be used to shape the content of our planning and team development retreat scheduled for June 17 and 18, 2010.

Please complete this by Monday, June 7, 2010. Your survey will go directly to the external consultant facilitating our session, so your replies will remain completely anonymous and confidential.
