GREY BRUCE HURON PERTH DHC Council Evaluation Questionnaire


This questionnaire is designed to assist Council members of the Grey Bruce Huron Perth DHC examine their approaches to governance and identify possible refinements.

All questionnaires will go directly to an external consultant so your replies will be completely anonymous and confidential. For the survey to be effective, it should be completed by all current members of the Council.

Findings will be presented and discussed at the October 27 Council retreat.

You will need to have have your Governance Manual handy in order to complete Question 5.
