CMHA Sudbury Branch Strategic Planning Process


The Canadian Mental Health Association, Sudbury Branch (CMHA Sudbury) has initiated a strategic planning process to identify future directions and priorities. Part of this process involves obtaining the views of our staff about the various issues and choices facing our agency.

Please complete this questionnaire by Friday, January 14, 2005. All questionnaires go directly to the external planning consultant helping us with the project, so your replies will be completely anonymous and confidential.

The survey findings will be used at a planning session with staff and Board members to be held on January 28th. You are all invited to attend this session.

To advocate for shared community responsibility by providing a continuum of services that reflects our vision for all individuals and families. To promote mental wellness and to eliminate discrimination associated with mental health for everyone in our district.
