Community Living York South
Strategic Planning Process

Community Living York South (CLYS) has initiated a strategic planning process to update our strategic plan and identify future directions and priorities. Part of this process involves obtaining the views of staff about the various issues and choices facing our agency.

Please complete the following questionnaire by Friday, April 11, 2008. You will have a chance to edit your responses when you click "Submit". Your survey will then go directly to the external planning consultant helping us with the project, so your replies will be completely anonymous and confidential. The survey findings will be used at our planning day on May 3.
