ALBERTA HEALTH SERVICES
ES Management Team Training Needs Assessment

This questionnaire is being distributed to all members of the Environmental Services (ES) Management Team at Alberta Health Services in order to get a better idea of our skills and knowledge base in the areas of infection prevention and control (IPC), occupational health and safety (OHS) and waste management. Findings will be used to plan for future management training and development opportunities.

Please complete this on-line questionnaire by Friday, May 21, 2010. Your survey will go directly to the external consultant helping us with the project so you can be frank and honest in your answers.

There are 36 questions so allow about 30 to 45 minutes to complete the questionnaire. You can stop working on it and return to the link at a different time if that is necessary. It is important that you answer the items on your own without discussion with your staff or colleagues. If you do not know the answer to a particular question, leave it blank.
